Up to one year, after which you must reapply to continue enrollment in the program |
Complete the appropriate form(s) with your doctor and then submit it by fax or mail with a valid prescription from your doctor, along with a tax return or other financial documentation. You should receive notification of your applications status within 10 days of receipt.

Takeda participates in Together Rx Access®, a free prescriptions savings program for eligible individuals and families who lack prescription drug coverage and are not eligible for Medicare.
